leave the room every now and then...
I am aware that might sound somewhat strange but I have noticed that leaving the room every once in a while is very helpful to make students talk without requesting the teacher's help so often.... Whenever I have a pair or a group practice and AFTER I monitor the students and see they really understand what they are supposed to do, I leave the room to grab a cup of coffee or to have some water... I don't let them notice I've left the room, and when I come back, they perform really well. That works out with adults and some teenagers as well. Let me know how it works for you.
